Principal Function: Under the direction of the Nurse Manager and the Maternal Health Nurse Care Coordinator, the Maternal and Child Health Navigator/Community Health Worker will work with underserved communities to provide resources for healthcare and maternal child services. Duties are aimed at building relationships with families. Community Health Worker needs to build trust and have good communication skills. They will provide family outreach, screenings, navigation, referral follow up and connection to resources as needed for maternal and child health. Collaborate with healthcare providers and community resources to connect individuals with the needed services, medical care, social services, and emergency assistance. Gather and relay data back to the healthcare organization.

Education and/or Experience: High school diploma or general education degree (GED), or one-year related experience and/or training; or equivalent combination of education and experience. A Community Health Worker certification is required.
