MSW Social Worker - Horizon Hospice of ColoradoHorizon Hospice is seeking a compassionate and dedicated Master of Social Work (MSW) professional to join our team.Pay Per Visit Structure: Starting at $50/visitPosition Overview: As an MSW Social Worker, you will report directly to the Director of Clinical Services and play a vital role in supporting patients and families through the hospice journey.Key ResponsibilitiesConduct initial psychosocial, emotional, spiritual, and bereavement assessmentsDevelop and implement goal-directed interdisciplinary care plansProvide ongoing counseling and crisis intervention for patients and familiesParticipate in IDG (Interdisciplinary Group) meetings and bereavement team activitiesAdvocate for patient and family needsCoordinate with community resources and agenciesMaintain accurate, timely documentation of all patient interactionsSupport patients and families with end-of-life care and bereavement needsQualifications & SkillsAbility to work independently and make sound clinical judgmentsStrong interpersonal and communication skillsEmotional resilience and ability to manage stress in sensitive situationsFlexibility, innovation, and compassion in care deliveryPhysical & Working ConditionsMinimal physical exertion (walking, reaching, lifting assistance up to 100 lbs)Exposure to clinical environments (including bodily fluids and varying conditions)Ability to adapt to temperature and environmental changesTransportation RequirementsValid driver's licenseReliable transportation and auto liability insuranceJoin us in making a meaningful difference in the lives of patients and their families.
