Hartford HealthCare is seeking an Advanced Practice Provider to join our Clinical Integration team. The Manager provides clinical, operational, and strategic leadership to advance value-based care performance, risk adjustment accuracy, quality improvement, and documentation integrity across the Clinically Integrated Network (CIN). In partnership with the Medical Director for Risk Adjustment, this role oversees the Risk Adjustment Coding and Education team, drives provider engagement, ensures regulatory compliance, and promotes best practices in clinical documentation, coding accuracy, and quality measure performance. In partnership with the Medical Director of Value Based Care, the Manager collaborates with clinical, operational, and administrative leaders to improve population health outcomes, optimize financial performance under value-based contracts, and strengthen quality and risk-adjusted performance metrics. Through education, incentive plans, analytics, and provider partnership, this role advances the organization''s strategic goals by engaging all providers in the Integrated Care Program (ICP) and fostering a culture of accountability, continuous improvement, and high-quality, patient-centered care.

Position Highlights

Clinical Documentation & Coding Oversight

• Oversight of Risk Adjustment Coding and Education team

• Documentation integrity initiatives

• Coding accuracy and risk capture improvement

• Quality measure documentation support

• Collaboration with CDI, coding, and operational leaders

Strategic Leadership

• Development and execution of Clinically Integrated Network value-based care strategies

• Alignment of risk, quality, and financial performance goals

• Provider incentive program design and oversight

• Population health and contract performance initiatives

• Partnership with Medical Directors and operational leadership

Clinician Engagement & Education

• Provider education and coaching

• Physician and APP champion development

• Departmental and specialty-specific engagement strategies

• Performance reviews utilizing scorecards and dashboards

• Change management and adoption of value-based care initiatives

• Communication of quality, risk, and incentive opportunities

Quality, Analytics & Performance Improvement

• Monitoring and reporting of quality and risk-adjustment KPIs

• Gap analysis and performance improvement planning

• Benchmarking and trend analysis

• Data-driven interventions to improve quality outcomes and financial performance

Compliance & Regulatory Oversight

• CMS, MSSP, Medicare Advantage, and payer compliance

• Audit preparedness and monitoring

• Regulatory education and risk mitigation

• Documentation and coding compliance oversight

Qualifications
