    Job Description :

    GENERAL OVERVIEW:

    This job is responsible for planning, organizing, directing, and providing safe and appropriate pharmacy services in accordance with current pharmacy law and accreditation standards. Responsible for the coordination of patient services with the nursing, delivery, and reimbursement departments.

    ESSENTIAL RESPONSIBILITIES:

    • Daily monitoring, assessment, and documentation of patients' lab work and clinical status.
    • Make recommendations for therapy changes when needed, based upon cultures and patient response to therapy.
    • Select best drug delivery method for new patients.
    • Discuss patient therapy with patient as needed.
    • Maintain pharmacy pathways.
    • Supervision of all Pharmacy staff including training and development of Pharmacists and Pharmacy Technicians.
    • Admixture and dispensing of medication and parenteral solutions to patients in alternate care settings, including formula/medication calculations and dispensing records.
    • Dispense pharmaceutical supplies as appropriate to patients in alternate care settings.
    • Maintain quality control requirements.
    • Accurate, timely recordkeeping.
    • Direct patient/caregiver teaching/care as appropriate for Pharmacy Services patients.
    • Direct physician contact (i.e., patient care communications, marketing).
    • Coordinate medication and supply deliveries to patients.
    • Provide information regarding drugs and their safe and appropriate use to other health care providers involved in patient's/client's care.
    • Actively participate in ongoing interdisciplinary communication specific to each patient.
    • Participate in the Performance Improvement program.
    • Participate in in-service programs for other health care professionals and for the community.
    • Attend seminars/CEU opportunities on an ongoing basis to promote growth and effective performance.
    • Thorough knowledge of and strict adherence to Allegheny Health Network Home Infusion's Mission, Vision, and Core Values.
    • Perform other duties assigned by management.

    QUALIFICATIONS:

    Minimum

    • Graduate of an accredited School of Pharmacy.
    • Licensed or eligible for licensure as a Registered Pharmacist in the State of PA at hire or sit for boards and obtain licensure within 120 days.
    • Post-graduate residency program in clinical pharmacy services.
    • Experience with or knowledge of admixture procedures, two years hospital or one year home care practice.
    • Supervisory experience.
    • Proof of current automobile insurance.

    Additional Employment Requirements:

    • Current valid driver's license.
