The Manager of Provider Transition and Growth, is responsible for leading and overseeing all membership movement activities associated with Primary Care Provider departures, retirements, panel updates, site closures, and strategic provider realignment initiatives across the Optum CA market. This role ensures continuity of care, revenue protection, regulatory compliance, and member retention during high impact transition events.

This position operates with significant autonomy and serves as an operational leader responsible for complex transition strategy development, cross functional coordination, workflow standardization, and performance reporting to regional and market leadership

This role directly influences membership retention, revenue continuity, provider alignment strategy, and market growth initiatives. Effective performance reduces financial leakage, mitigates compliance exposure, and supports long term strategic provider relationships.

Primary Responsibilities:

Membership Transition Leadership

  • Lead all membership activities related to PCP departures, retirements, site closures, panel reductions, and provider updates
  • Develop and execute transition strategies to ensure continuity of care and minimize member abrasion
  • Drive complex transition initiatives to completion, including nonstandard or high risk membership movement scenarios
  • Provide subject matter expertise on complex cases requiring strategic resolution

Strategic Problem Solving and Execution

  • Design collaborative solutions for nonstandard membership movement requests across multiple regions
  • Anticipate operational risk and proactively implement mitigation strategies
  • Ensure transition execution aligns with compliance standards, growth strategy, and retention objectives

Cross Functional Collaboration

  • Partner extensively with Operations, Provider Network, Market Leadership, Compliance, and external stakeholders to ensure accurate and timely membership movement
  • Build and maintain solid working relationships across internal and external stakeholders to protect membership stability
  • Serve as a trusted advisor to regional leadership during site closures, provider exits, and strategic realignment initiatives

Standardization and Process Development

  • Create and implement standardized workflows and best practices for PCP departures across the Optum CA market
  • Continuously evaluate and improve transition processes to enhance efficiency, reduce rework, and mitigate member disruption
  • Develop scalable frameworks to support growth initiatives and exclusivity strategies

Training and Team Development

  • Develop training curriculum and onboarding documentation related to membership activities and transition processes
  • Mentor and develop team members to strengthen operational knowledge and transition execution capabilities
  • Support organizational understanding of growth strategies and membership outreach processes

Performance Reporting and Executive Communication

  • Prepare and present regional and market level performance reports to senior leadership
  • Track and report project milestones, transition cycle time, retention impact, and operational outcomes
  • Provide executive level summaries and risk assessments for high visibility transition initiatives

Required Qualifications:

  • Proven experience managing complex healthcare membership transitions or provider network changes
  • Demonstrated ability to lead high impact, cross functional initiatives to completion
  • Proven solid analytical, organizational, and problem-solving skills
  • Ability to operate independently with executive level communication capability
  • Experience developing workflows, training materials, and standardized operational processes
  • Driver's License and access to reliable transportation

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you'll find a far-reaching choice of benefits and incentives. The salary for this role will range from $72,800 to $130,000annually based on full-time employment. We comply with all minimum wage laws as applicable.
