About the Role:

The Manager of Intensive Supports is responsible for overseeing and coordinating services for individuals with developmental disabilities who require intensive, 24/7 care and/or have complex needs. This role focuses on providing leadership, planning, and supervision of support services for individuals, including those receiving supervised living, staffed living, and in-home support, including those under ACT 248. The Manager of Intensive Supports ensures individuals receive the highest quality care in the least restrictive environment, working collaboratively with internal and external teams to meet their unique needs.

Principal Responsibilities:

  • Plan, develop, and manage services for individuals receiving intensive support, including supervised living, staffed living, in home, and other direct support for individuals under ACT 248.
  • Uphold and promote Community Care Network (CCN) values throughout the Intensive Supports Team to ensure high-quality, person-centered care.
  • Collaborate with teams within Developmental Services (DS), CCN, and external agencies to ensure the needs of individuals are met effectively and efficiently.
  • Coordinate appropriate staff coverage and develop staff schedules with the DS Scheduler, utilizing internal and contracted staffing resources as needed, to ensure continuity of services and maintenance of productivity targets.
  • Coordinate and oversee contracted staffing services from staffing agency to support adequate coverage and continuity of care. Serve as a primary program contact with staffing agency with contracted staffing services regarding staffing needs, scheduling concerns, performance issues, training needs, documentation expectations, and changes in individual support needs.
  • Ensure contracted staff receive the individual-specific information, orientation, and training necessary to provide services safely and effectively, and collaborate with contracted staffing services leadership to address staffing, performance, training, and documentation concerns in a timely manner.
  • Coordinate and support transitions from Shared Living Provider (SLP) homes into Staffed Living, working with individuals, families/guardians, providers, clinical teams, and DS staff to promote continuity, stability, and successful transitions.
  • Develop and oversee transition plans for individuals entering Staffed Living, ensuring appropriate staffing, staff readiness, training, documentation, environmental needs, and individualized supports are in place before and following transition.
  • Ensure team members receive required and individual-specific training to safely and effectively implement person-centered programs, support plans, and strategies that enhance independence and community living skills.
  • Provide training to team members to implement person-centered programs aimed at enhancing independent living skills.
  • Monitor and ensure that team documentation is completed accurately and within required timeframes, including service documentation, progress notes, incident reporting, and other required records, in compliance with Department of Disabilities, Aging and Independent Living (DAIL), CCN, and Medicaid guidelines.
  • Monitor and ensure that the quality of life of individuals is maintained or improved, and that their rights are protected.
  • Pursue the least restrictive environments for individuals by assisting teams in identifying cost-effective, independent housing options.
  • Build and maintain community partnerships with landlords in Rutland County to create affordable, independent housing options, in collaboration with the Housing Coordinator.
  • Collaborate with the Shared Living Manager, Housing Coordinator, individuals, families/guardians, and other team members when an individual is transitioning from a Shared Living Provider (SLP) home or when the Shared Living model no longer meets the individual's needs.
  • Maintain familiarity and relationships with individuals supported to ensure continuity of care and individualized attention.
  • Foster and maintain collaborative relationships with families, guardians, community members, and state partners to support holistic care.
  • Participate in an on-call rotation and provide after-hours consultation, direction, and response for Intensive Supports, including staffing emergencies, crisis situations, and other urgent service needs; provide in-person management or direct support when circumstances require.
  • Provide leadership and hands-on support during crisis situations, assisting staff and individuals with stabilization, problem-solving, implementation of established crisis/support plans, and coordination with internal and external resources as appropriate.
  • Assist with quality assurance and compliance activities to monitor and improve service delivery.
  • Ensure that the program operates within the specified budget guidelines and contributes to financial sustainability.
  • Provide clear expectations, ongoing support, and constructive feedback to direct reports.
  • Ensure staff training and the implementation of new policies, procedures, and guidelines; fostering a culture of continuous learning.
  • Conduct timely performance evaluations for staff under supervision.
  • Hold regular staff/team meetings to promote communication, collaboration, and professional development.
  • Participate in the recruitment and hiring process for staff, providers, and external contracts to ensure the programs staffing needs are met.
  • Perform other duties as assigned to support the overall success of the program.

Qualifications:

  • Education: Bachelor's degree in a related field preferred; equivalent experience will be considered.
  • Experience: Minimum of 4 years of experience in developmental disabilities services, with experience in intensive supports or similar services preferred.
  • Certification: Qualified Developmental Disabilities Professional (QDDP).
  • Leadership: Previous leadership or management experience required, with strong leadership skills, including accountability, follow-through, coaching, and constructive feedback.
  • Technical Skills: Proficiency with computers, including Microsoft teams and experience entering data into Electronic Medical Records (EMR) systems.
  • Collaboration: Ability to work collaboratively and professionally with teams, families, individuals, and external partners.
  • Communication: Strong writing, organizational, and communication skills to convey information effectively and efficiently.
  • Adaptability: Flexibility, dependability, and the ability to work independently and adapt to changing needs.
  • Crisis Management: Ability to respond appropriately in emergency situations and manage crises effectively.
  • Commitment: Strong commitment to person-centered services, community inclusion, and empowering individuals to make their own choices.
  • Other Requirements:
  • Ability to adjust work schedule as needed to meet program demands.
  • Must have a valid driver's license, reliable transportation, and personal automobile liability insurance coverage at or above state-mandated levels.
  • Successful completion of a criminal history background check after hire.
