This leadership role oversees case management, utilization review, and discharge planning operations within a 24/7 clinical environment.

This is a high impact position for a leader who excels in care coordination, regulatory compliance, team development, and driving measurable improvements in patient flow and outcomes.

Role Overview

The Manager provides operational, clinical, and administrative leadership for case management and social work services. This includes oversight of staffing, performance management, quality initiatives, and daily workflow operations. The role partners closely with senior leadership to strengthen processes, elevate care coordination, and support organizational goals.

Key ResponsibilitiesLeadership & Department Operations
  • Lead, mentor, and support case management and social work teams.

  • Oversee staffing assignments, productivity expectations, and labor management.

  • Ensure compliance with all regulatory and accreditation standards.

  • Maintain effective communication across all shifts and interdisciplinary teams.

  • Develop and implement policies, procedures, and performance standards.

Clinical & Utilization Management
  • Provide guidance on acute case management, care coordination, and discharge planning.

  • Review cases, documentation, and utilization review findings.

  • Monitor and improve key metrics such as LOS, readmissions, observation stays, and avoidable delays.

  • Serve as a clinical resource for staff regarding assessments, care plans, and medical necessity.

Quality & Performance Improvement
  • Develop and monitor departmental quality indicators.

  • Conduct audits and implement action plans to improve patient flow and care coordination.

  • Support continuous quality improvement, benchmarking, and outcomes measurement.

People Management
  • Recruit, develop, coach, and evaluate staff.

  • Ensure timely completion of performance evaluations.

  • Provide orientation, in service training, and ongoing competency development.

  • Address attendance, scheduling, and performance issues, including oversight of timekeeping systems.

QualificationsLicensure & Education
  • Active RN license (state specific).

  • Bachelor's degree in Nursing required; Master's degree preferred.

  • BLS certification required.

  • National case management certification preferred.

Experience
  • Leadership experience in acute care case management required.

  • Experience in utilization management and discharge planning.

  • Background supporting patients with complex medical, emotional, and social needs.

  • Experience leading teams, managing workflows, and driving operational improvements.

Skills
  • Strong knowledge of CMS, Title 22, Joint Commission, and case management standards.

  • Proficiency with InterQual, Meditech, and managed care software.

  • Excellent communication, collaboration, and problem solving abilities.

  • Ability to lead diverse clinical teams and manage competing priorities.
