    Manages and coordinates the day-to-day operations of the department and workflows of the case management team.  This role promotes effective utilization and monitoring of health care resources, provides a leadership role to support clinical, financial and resource outcomes.  Responsible for leading the discharge planning function and supporting the Hospital’s strategic initiatives and outcomes. Influences policies that achieve health equity, promote wellness, and protect against inequity for people of marginalized and underrepresented communities.

    Education/Training: Bachelor’s Degree required, RN preferred.  Ability to read, write and communicate in English

    Experience:  Minimum of three (3) years related experience in a health care setting with at least one (1) year of supervisory or management experience.  Demonstrates understanding of assessment, planning and managing patient care.  Evidence of advanced communications and negotiating skills and sensitivity to customer needs. Familiarity with all state and regulatory requirements related to case management and social work.  Understanding of the role of both the case manager and the social worker in the hospital setting.  Evidence of leadership, creativity, integrity and initiative.  Ability to interface with wide variety of individuals and handle several complex situations simultaneously. 

    Licensure, Registration or Certification: Current New York State Licensure as a Registered Professional Nurse or Social Worker.  Certification in Case Management, PRI Certification preferred.
