Rate of Pay: $36.00 - $39.00 per hour

Schedule: Saturday & Sunday | 7am-3:30pm

Additional pay differentials of $1.25 per hour for the PM shift and $1.50 per hour for the NOC shift

Who you are:

  • LVN with two years of geriatric nursing/assessment experience.
  • One year of dementia care experience preferred.
  • Strong communication and documentation skills.
  • Ability to assess resident needs and respond appropriately to changes or emergencies.
  • Ability to provide clear direction to Resident Helpers and work effectively as part of a team.
  • Compassionate, professional, and resident-focused approach to care.

Job Summary:

The SCR Licensed Vocational Nurse (LVN) has direct oversight, implementation and evaluation of all dementia initiatives. The primary responsibility is providing a therapeutic environment for the Palmer Special Care Residence to maintain each resident's highest level of physical, social and physiological well-being. The Charge SCR LVN provides resident validation and effective redirection, anticipates the residents' needs and have effective communication with the cognitively impaired.

Essential Job Duties

  • Orders and maintains medications.
  • Keeps accurate maintenance of medical records.
  • Schedules necessary service for residents on a daily basis.
  • Coordinates with physicians and health agencies regarding the needs for Physical/Occupational Therapy, Lab works, and other special services.
  • Assists with scheduling doctors' appointments and transport requests if necessary.
  • During emergencies, takes proper action in accordance with approved emergency manual and notifies designated personnel.
  • Responsible for completion and e-mailing of shift reports.
  • Attends all mandatory facility in-service programs and any other mandatory meetings required by management.
  • Attends Residential Health Care and SCR Department meetings as scheduled.
  • Attends regular, on-going training sessions scheduled by the Director of Staff Development and Director of AL/SCR.
  • Assists in the effective operation of Casa de las Campanas and participates in relevant facility activities.
  • Responsible for assisting Director in the evaluating of SCR CNAs.
  • Coordinates discharge of resident from Health Center back to SCR and schedules follow-up care.
  • Ability to work as a member of a team and cooperate with co-workers.
  • Contributes to a positive, healthy and safe working environment and accepts change in a positive manner.
  • Performs any other relative, relevant duties as assigned per managers' request.
