    Licensed Vocational Nurse (LVN) Care CoordinatorRole Summary

    Empower patients and drive clinical excellence as a Licensed Vocational Nurse Care Coordinator. In this vital role, you will act as a bridge between high-risk patient populations and their primary care providers, ensuring that no one falls through the cracks of the healthcare system. You will be at the heart of value-based care, focusing on quality improvement initiatives and managing care for patients with complex, multi-morbid conditions. This position offers the unique opportunity to influence patient outcomes directly through proactive outreach, health education, and the closing of critical care gaps.

    Key Duties:

    • Proactive Patient Outreach: Conduct telephonic outreach to members to close care gaps, address medication adherence, and facilitate improved star ratings and quality performance.

    • Network Support: Support ACO and CIN network providers in meeting quality improvement initiatives and monitoring high standards of care.

    • Clinical Gap Identification: Pull reports to identify open care gaps and participate actively in targeted Quality Improvement Programs.

    • Provider Collaboration: Maintain ongoing, seamless communication with healthcare providers through various reporting tools and multidisciplinary meetings.

    • Patient Advocacy: Support Primary Care Providers by assisting patients in scheduling preventive screenings and essential follow-up appointments.

    • Health Education: Provide expert counseling and health education to patients and their families using standardized protocols and engaging materials.

    • Subject Matter Expertise: Serve as a knowledgeable resource in care transitions and quality metrics, assisting in the education of clinical staff and stakeholders.

    • Leadership Excellence: Meet organizational competencies by serving as a leader of self and others in a high-performance clinical environment.


    Required Qualifications
    • Licensure: Must hold a current, valid Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) license.

    • Experience: Minimum of 3 years of clinical or home health experience is required to ensure a strong foundation in patient care.

    • Residency: Must currently reside in the state of Texas.

    • Education: High School Diploma or equivalent required (proof of education must be submitted).

    • Compliance: Must provide a NURSYS license verification, including documentation regarding any license disciplines.


    Desired Qualifications
    • Communication: Exceptional telephonic communication skills and the ability to build rapport with diverse patient populations.

    • Technical Savvy: Proficiency in pulling clinical reports and navigating healthcare databases to identify quality metrics.

    • Care Coordination: Prior experience in value-based care, ACO environments, or managing high-risk patient populations is highly beneficial.
