LVN – Home Health | Day Shift (5 x 8s)

Job Highlights

• Day Shift: 5 x 8-hour shifts

• Mileage Reimbursement: Paid in addition to compensation

• EMR: Must have experience with HCHB and Epic

• Provide skilled nursing care to patients in a home health setting under the direction of an RN or MD.

• Participate in patient care planning and treatment.

• Perform sterile procedures and other nursing treatments using proper aseptic technique.

• May provide guidance to ancillary staff.

• On-call responsibilities may be required based on agency needs.

Minimum Requirements

• Current LVN/LPN License in the state of practice

• BLS Certification

• Experience with HCHB and Epic (Required)

• Home Health experience preferred

• Valid Driver's License (Preferred)

• Arrhythmia certification (Preferred)
