Position: RN Case Manager
Location: Fort Walton Beach, FL
Schedule: Full-Time | Days (8:00 AM – 4:30 PM) | Rotating Weekends

This role combines clinical expertise, patient advocacy, discharge planning, and care coordination to ensure safe, effective, and compassionate outcomes.

This is an excellent opportunity for nurses with acute care experience who are passionate about patient-centered care and interested in advancing into or growing within Case Management.

Key Responsibilities

• Coordinate patient care plans from admission through discharge
• Conduct comprehensive psychosocial and discharge planning assessments
• Collaborate with physicians, nurses, therapists, and interdisciplinary teams to ensure optimal patient outcomes
• Educate patients, families, and caregivers regarding treatment plans, discharge expectations, insurance benefits, and available resources
• Advocate for patients and support safe, appropriate discharge planning

Qualifications

• Active Registered Nurse (RN) license
• Associate Degree in Nursing or higher
• Minimum 2 years of recent Acute Care experience within the last 3 years
• Experience in ICU, CCU, Med-Surg, LTAC, or Case Management preferred

Compensation & Benefits

• $34 – $47 per hour
