    Description

    This position is Private Duty Homecare. We offer 12-hour shifts. 3 days a week.

    JOB DESCRIPTION:
    POSITION TITLE: Licensed Practical Nurse.
    REPORTS TO: Registered Nurse.

    SCOPE OF SERVICES: Services shall be provided in accordance with the scope of nursing practice laws and associated rules, and the client service plan.

    QUALIFICATIONS:
    • A current license to practice as a Licensed Practical Nurse in the State of Georgia.
    • Two years’ experience in a home health services or related field.
    • Knowledge of current methods of home management, the provision of personal care, the ability to organize and coordinate the work of others.
    • Evaluate different situations and make decisions based on client care and health.
    • Communicate and deal effectively with others.
    • Must have a working knowledge of Chapter 290-5-54 Rules and Regulations, “Nursing Services” Private Home Care.
    • A satisfactory criminal background check.

    DUTIES:
    • Regularly assess the nursing needs of the client.
    • Provide nursing services as needed and in accordance with the client’s service plan.
    • Reviewing the content of the Service Plan at least every two months (every 60-62 calendar days) or as needed.
    • Supervises personal support aides.
    • Report problems and progress of client to supervisory personnel or the client’s personal physician.
    • Conduct face-to face evaluations in the client’s home (with client or responsible caregiver) at least every two months (every 60-62 days) or as needed to re-evaluate the needs of those clients whose health status and situation involves complex situations.
    • Educate clients, client’s representative and/or caregivers regarding the client’s health, level of functioning and health care needs.
    • Completing timely pertinent supervisory notes.
