About the Role:We Care For You Senior Services is looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join our growing team in Southbury, CT. Responsibilities:- Provide direct patient care to senior residents in accordance with established care plans and clinical protocols
- Administer medications and treatments as prescribed, ensuring accurate documentation
- Monitor and record patient vital signs, symptoms, and changes in condition
- Collaborate with RNs, physicians, and interdisciplinary care teams to coordinate resident care
- Assist with wound care, catheter care, and other skilled nursing procedures
- Communicate effectively with residents, families, and caregivers regarding care plans and health updates
- Maintain accurate and timely medical records in compliance with state and federal regulations
Requirements:- Active LPN license in the state of Connecticut (required)
- Previous experience in geriatric, long-term care, or senior services settings preferred
- Strong knowledge of medication administration and clinical nursing procedures
- Excellent communication, organizational, and critical thinking skills
- Compassionate, patient-centered approach to elder care
- Ability to work independently as well as collaboratively within a care team
- CPR/BLS certification preferred
