About the Role:
Candis Inc in Roanoke, VA is looking for a dedicated and compassionate Licensed Practical Nurse (LPN) to join their growing healthcare team. This is an exciting opportunity to make a real difference in patients' lives while advancing your nursing career in a supportive environment.



Responsibilities:

• Provide direct patient care including monitoring vital signs, administering medications, and documenting health status
• Assist in developing and implementing individualized patient care plans
• Collaborate with RNs, physicians, and interdisciplinary team members to ensure quality care
• Perform wound care, catheter care, and other clinical nursing procedures
• Educate patients and families on health conditions, treatments, and discharge instructions
• Accurately maintain and update electronic health records (EHR) and patient documentation
• Respond promptly to patient needs and changes in condition

Requirements:

• Active LPN license in the state of Virginia (or eligibility to obtain)
• Graduate of an accredited Licensed Practical Nursing program
• Current CPR/BLS certification
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal skills with patients and care teams
• Ability to work independently and in a team-oriented environment
• Prior LPN experience preferred but new graduates are encouraged to apply
• Experienced in Mental Health/Behaviors
