At Holy Family, we care like family.What You'll Do as an LPN Resident Care Coordinator:Oversee medication administration, providing guidance and supervision to Medication Aides and ensuring medications are administered and documented according to regulations and facility policies.Provide clinical oversight and supervision to Caregivers, helping ensure residents receive safe, consistent, high-quality care.Complete resident assessments and nursing documentation, evaluating residents' health needs and changes in functional status and maintaining required clinical records.Receive, transcribe, and implement physician orders, ensuring provider orders are carried out safely and promptly and administering medications or routine injections as needed.Respond to resident health needs and clinical emergencies, assessing residents, providing appropriate nursing interventions, addressing safety concerns, and coordinating additional medical services when necessary.Coordinate resident care and communication with residents, families, providers, and staff while supporting continuity of care, resident preferences, dignity, independence, and choice.LPN Resident Care Coordinator Qualifications: Current Licensed Practical Nurse (LPN) license or eligibility for licensure in PennsylvaniaCurrent CPR certification or ability to obtainHigh School Diploma or GED1+ year of clinical healthcare experience preferred, ideally in senior living, personal care, nursing, acute care, home care, or hospiceStrong verbal and written communication skills with the ability to effectively communicate with residents, families, providers, and team membersAbility to meet the physical requirements of the positionComfortable working in a pet-friendly environment Why Holy Family Senior Living?Meaningful work that makes a difference in residents' lives every dayA compassionate, family-centered workplaceAbout Holy Family Senior LivingLocated on a beautiful 20-acre campus in Bethlehem, PA, Holy Family Senior Living has served older adults and their families since 1882.
