Hours: 3x8 hr PT Day/Evening shifts (variable, scheduled as needed), including EOW & holidays
Provide direct and indirect nursing care under RN supervision
Assist with resident ADLs, safety, nutrition, and mobility
Document care and report changes in resident condition
Follow infection control and safety protocols
NY LPN Licensure (Required)
BLS Certification (AHA or RQI) (Required)
Minimum 1 year LPN experience in LTC or CNA capacity (Required)
High School Diploma or GED
