Job Description

The LPN Intake Specialist manages and facilitates timely transfer of patients to home health, DME and infusion. This is accomplished by receiving referrals and physician's orders from socials services, case managers, nurses, physicians as well as through Valley Health's hospital EMR. The information is then documented in the home health electronic medical record and communicated to the appropriate personnel. In addition, the LPN Intake Specialist performs ICD coding for all referrals and obtains all necessary face to face documentation whether internal to Valley Health through EPIC or via outside referrals.

Experience

Experience in Home Health and/or medical/surgical nursing is preferred.

Certification & Licensures

LPN license or license-eligible in Virginia and/or West Virginia required, based on state of work assignment.

Qualifications

Strong interpersonal skills and an outgoing personality are required.

Strong reading, writing, spelling, grammar, punctuation and mathematical calculation skills.
