LPN – Night Shift
Location: Oneida, NY 13421
Contract: 14 Weeks
Shift: Nights
Hours: 36 Hours/Week
Job Summary
We are seeking an experienced Licensed Practical Nurse (LPN) to provide compassionate, high-quality care to residents at a Rehabilitation and Extended Care Facility in Oneida, NY. The LPN will work closely with Registered Nurses, physicians, CNAs, Unit Managers, and other members of the interdisciplinary team to ensure residents receive safe and effective care.
Key Responsibilities
Provide compassionate nursing care to residents while maintaining a professional and customer-focused approach.
Monitor residents and promptly report changes in condition to the RN and physician.
Collaborate with the interdisciplinary team to meet residents' daily care needs.
Administer medications and treatments according to established orders and facility policies.
Assist with wound care, equipment preparation, and other nursing procedures as needed.
Delegate appropriate tasks to CNAs and other support staff and monitor the quality of care provided.
Maintain accurate clinical documentation and enter orders into the electronic medical record.
Coordinate resident appointments and communicate appointment and transportation details to families.
Communicate with pharmacies regarding medication orders and refills.
Participate in care planning, discharge planning, staff meetings, and resident care conferences.
Maintain compliance with controlled-substance procedures and all facility policies.
Maintain a clean, safe, and healthy environment for residents.
Report incidents involving suspected abuse, neglect, mistreatment, or exploitation.
Respond appropriately to building emergencies and other safety concerns.
Qualifications
High school diploma or equivalent.
Completion of an accredited Licensed Practical Nurse program.
Active, unencumbered New York LPN license required.
Minimum 2 years of LPN experience required.
Long-term care, skilled nursing, rehabilitation, or extended-care experience preferred.
BLS/CPR certification preferred.
Strong communication, teamwork, and clinical judgment skills.
Compassionate and patient-centered approach to care.
Ability to work effectively in a fast-paced healthcare environment.
Physical Requirements
Ability to lift and carry objects weighing 45 pounds or more.
Ability to push and pull equipment or other objects.
Ability to stand, walk, and sit for extended periods.
Ability to bend, kneel, reach above shoulder level, and perform repetitive movements.
Good hand-eye coordination and ability to perform routine nursing tasks.
Ability to operate a motor vehicle when required.

2 years of LPN experience: Required
Unencumbered New York LPN license: Required
