    MDS Coordinator Assistant (LPN)

    Join Our Team

    We're seeking a detail-oriented and compassionate Licensed Practical Nurse (LPN) to join our Long-Term Care team as an MDS Coordinator. In this role, you'll help ensure the accuracy and timeliness of resident assessments while supporting quality care, regulatory compliance, and appropriate reimbursement. Working collaboratively with our interdisciplinary team, you'll play an essential role in developing individualized care plans that promote the best possible outcomes for our residents.

    Position Summary

    The MDS Coordinator is responsible for coordinating, completing, and maintaining Minimum Data Set (MDS) assessments in accordance with federal and New York State regulations. This position works closely with nursing, rehabilitation, social services, dietary, and other departments to ensure comprehensive resident assessments and person-centered care planning.

    Responsibilities

    • Coordinate and complete MDS assessments accurately and within required timeframes.
    • Ensure compliance with federal and New York State Department of Health regulations.
    • Collaborate with the interdisciplinary team to develop and update individualized care plans.
    • Monitor documentation to ensure accuracy, completeness, and regulatory compliance.
    • Support quality improvement initiatives related to resident assessments and outcomes.
    • Assist in preparation for regulatory surveys and audits.
    • Maintain confidentiality of resident information and documentation.
    • Communicate effectively with residents, families, providers, and interdisciplinary team members.
    • Prioritize multiple responsibilities while meeting established deadlines.
