LPN/LVN – Long-Term Care

Job Title: LPN / LVN
Location: 110 West 6th Street, Oswego, NY 13126
Contract Duration: 8 Weeks
Schedule: Day Shift
Hours: 36 Hours/Week
Shift Options: 6:00 AM–6:00 PM or 10:00 AM–10:00 PM
Weekend Requirement: Every Other Weekend

Position Overview

Seeking an experienced Licensed Practical Nurse (LPN/LVN) to provide nursing care to residents in a Long-Term Care setting. The primary patient population includes individuals with dementia and other chronic care needs.

Requirements

  • Minimum 1 year of LPN/LVN experience required.

  • Must hold an active New York LPN license.

  • BLS certification required.

  • Long-term care or skilled nursing experience preferred.

  • Must be willing to float as needed — no exceptions.

  • Must be available to work every other weekend.

  • Must be available for 36 hours per week throughout the 8-week contract.

Patient Population

  • Primarily long-term care residents.

  • Common diagnosis includes Dementia and related cognitive/functional impairments.

  • Typical nurse-to-patient ratios range from approximately 1:20 to 1:40, depending on unit needs and staffing.

Key Responsibilities

  • Provide direct nursing care to long-term care residents.

  • Perform assessments and monitor residents for changes in condition.

  • Administer medications and treatments as ordered.

  • Assist with care planning and communicate resident needs to the healthcare team.

  • Document patient care accurately in the medical record.

  • Provide compassionate care while maintaining resident safety and dignity.

  • Float to other units/floors as needed based on staffing requirements.
