    Requirements:

    • Active LPN license in Hart.

    • Provide direct nursing care under the supervision of an RN or physician
    • Administer medications and basic treatments according to care plans
    • Assist with daily living activities and resident hygiene
    • Observe and report changes in residents’ conditions
    • Accurately document nursing interventions and outcomes
    • Collaborate with the care team to support resident-centered care
