OneTeam Healthcare is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide care to residents within local retirement and senior living communities. This role offers the opportunity to enjoy the flexibility and autonomy of home health while minimizing travel and spending more time focused on patient care.

Responsibilities
  • Provide skilled nursing care under the direction of the RN and physician
  • Perform patient assessments and monitor condition changes
  • Administer medications and treatments as ordered
  • Assist with disease management and patient education
  • Document visits and care provided accurately in EMR system
  • Communicate effectively with interdisciplinary team, families, and providers
  • Promote patient comfort, safety, and independence
Qualifications
  • Active LPN license in good standing
  • Previous home health, assisted living, SNF, or senior care experience preferred
  • Strong communication and organizational skills
  • Compassionate, dependable, and patient-focused approach
  • Ability to work independently while collaborating with a clinical team
  • Current driver's license and reliable transportation
