Description:

Job Title: LPN - Home Health
Location: Portland, OR 97219
Duration: 3+ months with possibilities of extension

Shift & Schedule: 8:00 AM – 4:30 PM

• On Call/Weekend Requirement: Yes – on call weekends and evenings once monthly

Coverage Area: Portland / PDX area and surrounding (west side of river)

Responsibilities

• Daily home health nursing visits
• Assessment
• Education
• Wound care
• Heart failure management
• Medication management

Qualifications

• Minimum Education Requirement: Associates Degree
• Home health experience required
• Licensed Practical Nurse in the state of Oregon
• Occupational health screenings required

Preferred Skills

• Experience with Pointcare software in a home health setting
