LPN for Quality Improvement for Certified Home Health Agency (CHHA)

Job Overview
We are seeking a detail-oriented, knowledgeable, and motivated Licensed Practical Nurse (or RN), specializing in Home Care with a focus on compliance and chart review. This role involves ensuring accurate medical documentation, adherence to CMS (Centers for Medicare & Medicaid Services) Standards, and maintaining high-quality patient care through meticulous review processes. The ideal candidate will possess strong clinical knowledge, familiarity with medical coding, and experience in utilization/chart review within Certified Home Health Agency (CHHA) settings.

Duties

• Review and analyze OASIS (Outcome and Assessment Information Set) data for accuracy, completeness, and compliance with regulatory standards.
• Ability to review clinical documentation, visit notes, and plans of care for accuracy, medical necessity, and consistency.
• Conduct thorough audits of medical records to ensure documentation aligns with Medicare requirements and NCQA (National Committee for Quality Assurance) standards.
• Utilize EMR (Electronic Medical Record) systems such as HCHB (Home Care Home Base) to extract, review, and document clinical data efficiently.
• Collaborate with, educate and train clinical staff to verify proper documentation of patient assessments, and treatments.
• Apply knowledge of coding classifications for accurate medical management documentation.
• Support compliance initiatives by identifying documentation deficiencies and recommending corrective actions.
• Participate in ongoing education related to healthcare regulations, HIPAA privacy standards, and clinical documentation improvement strategies.
