Description

Licensed Practical Nurse (LPN)

Schedule: 6:00 AM – 6:00 PM
Employment: Full-Time or PRN
Shift: 12-Hour Day Shift | Block Scheduling

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide high-quality nursing care to residents in a supportive, team-oriented environment.

This position offers 12-hour day shifts from 6:00 AM to 6:00 PM, with both Full-Time and PRN opportunities available. Full-time staff work a rotating block schedule that provides two consecutive days off and every other weekend off.

Responsibilities

  • Administer medications and perform treatments as ordered by physicians and within the scope of your nursing license.

  • Provide medication through oral, enteral, parenteral, suppository, and other prescribed routes.

  • Perform treatments including sterile and aseptic wound care, postural drainage and percussion, tracheostomy care, gastrostomy and jejunostomy care, and catheter care.

  • Observe residents and report the effectiveness of medications and treatments.

  • Monitor and maintain adequate medication and medical supply levels.

  • Receive and communicate information regarding residents' medical, social, and functional needs.

  • Participate in shift reports to promote continuity of care.

  • Communicate effectively with residents, families, nursing staff, and interdisciplinary team members.

  • Complete initial nursing assessments for newly admitted residents.

  • Assist with MDS assessments, quarterly reviews, and individualized care plans.

  • Maintain accurate clinical and administrative documentation regarding diagnoses, observations, treatments, and resident progress.

  • Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.

  • Attend and participate in staff meetings.

  • Recognize changes in resident condition and respond appropriately to medical concerns and emergencies.

Qualifications

  • Current LPN license in good standing.

  • Knowledge of geriatric/gerontological nursing and the aging process.

  • Understanding of medication administration, pharmacology, and nursing treatment procedures.

  • Knowledge of applicable nursing regulations and medication safety requirements.

  • Ability to assist with the development and implementation of resident care plans.

  • Strong observation, assessment, documentation, and clinical decision-making skills.

  • Excellent written and verbal communication skills.

  • Ability to establish positive working relationships with residents, families, and interdisciplinary team members.

  • Ability to respond professionally and appropriately to medical emergencies.

Schedule

6:00 AM – 6:00 PM

  • 12-hour day shifts

  • Full-Time or PRN opportunities available

  • Rotating block schedule

  • Two consecutive days off

  • Every other weekend off
