We are seeking a full-time LPN to cover the Vandalia, Huber Heights, Fairborn, Yellow Springs, Xenia, and Beavercreek area and who has the heart and skills to work with home bound patients.

Responsibilities:

• Provide care wherever the patient calls home.
• Carry out the plan of care as directed by the patient's physician.
• Work with the physician for medication needs and changes to the plan of care.
• Ability to complete documentation within an EMR System. (DeVero)
• Help decrease the need for re-hospitalization by being actively involved in the patient's plan of care to assess any changes that may be needed.
• When appropriate, teach and counsel family members about care and process for disease/injuries.
• Attend orientation, meetings and CEUs,
