    Employment Type: Full-time

    Shift: Days

    Hours Per Week: 40 hours/week  

    Location Amsterdam Family Practice: Holland Circle Drive

    Hiring Range: LPN: Starting at $25.50 hourly. RN: Starting at $36.67 hourly

    Below are the shift differentials based on eligible shifts worked:

    • Weekday evening shift - additional 10%
    • Weekday night shift - additional 15%
    • Weekend day shift - additional 10%
    • Weekend evening & night shifts - additional 21%

    The actual compensation for this position will be determined based on experience and other factors permitted by law.

    Responsibilities for LPN:

    • Contributes to the assessment of patients and assures the well-being of patients.
    • Administers medications and observes patients for adverse reactions to medications or treatments.
    • Performs laboratory tests and therapeutic services.
    • Educates patients/families about disease treatment plan including self-care post discharge, holistic health needs, available resources and follow up care.

    Responsibilities for RN:

    • Perform clinical assessment of patient symptoms using standardized triage protocols, algorithms, and RN clinical judgment.
    • Evaluate patient symptoms, medical history, and concerns to determine the need for further intervention, appointments, or other (emergency care, urgent care, same‑day visit, routine appointment, or home care).
    • Coordinate and schedule same‑day, urgent, and walk‑in appointments based on clinical need
    • Escalate high‑risk, complex, or unclear cases to providers promptly.
    • Provide patient education, including home care guidance, chronic disease self‑management, and return‑to‑care instructions.
    • Accurately document assessments, advice, disposition, and follow‑up in the electronic health record (EHR).
    • Monitor patient flow to promote timely access to recommended services.
    • Collaborate with providers, Medical Assistants, LPNs, and front‑end staff to support efficient clinical workflows.
    • Adhere to established protocols, policies, and procedures to ensure patient safety and quality outcomes.
    • Provide education to patients on conditions, medications, lifestyle modifications, and preventative care.
    • Document patient interactions accurately and in a timely manner, ensuring compliance with clinical protocols.
    • Work collaboratively with the healthcare team to ensure patients receive the best care possible.

    Complexity of Work:
    • Within scope of job, requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment.

    Personal Protective Equipment:
    • Follows Standard Precautions using personal protective equipment as required.

    Licensure / Certification / Registration:

    Required Credential(s):

    • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date.
    • LPN requirements: Practical/Vocational Nurse credentialed from the New York Board of Nursing obtained prior to hire date or job transfer date. 
    • RN Requirements: Registered Nurse credentialed from the New York Board of Nursing.Diploma from an accredited school/college of nursing required.
