Weekly Guarantee: 32 hours

 Productivity Expectation: 30 points

Coverage Areas: Davidson/Williamson County

Mileage Reimbursement: Yes — reimbursed between visits, from first patient to last patient

Pay Rate: $30-$32/hr

About the Role

We are seeking a dedicated and clinically skilled Travel Licensed Practical Nurse (LPN) to provide high-quality home health and hospice services within the Davidson and Williamson County areas. In this role, you will work under the supervision of a Registered Nurse (RN) to deliver direct patient care, maintain rigorous documentation, and contribute to positive patient outcomes within a multidisciplinary team.

Key Responsibilities

• Clinical Care: Provide high-quality clinical services within the LPN scope of practice and according to the established plan of care.
• Observation: Continually monitor and report patient nursing needs and changes in condition to the supervising RN.
• Documentation: Thoroughly document visits per policy and payer requirements, maintaining accurate medication listings and syncing data timely via HCHB.
• Coordination: Communicate effectively with physicians, family members, and agency staff regarding visit assignments, schedule changes, and care coordination.
• Quality Improvement: Participate in performance improvement plans to reduce hospitalizations and enhance patient outcomes.
• Professionalism: Attend all required meetings and case conferences punctually; maintain a positive, adaptable attitude toward patient needs and agency goals.

 Schedule & On‑Call

• Shift Schedule: Mon-Fri 8:00am-4:30pm
• On‑Call/Weekend Requirements: 1 week Monday-Sunday every 6 weeks

 Required Qualifications

• Experience: Previous experience in Home Health is required.
• Technology: Proficiency with Homecare Homebase (HCHB) EMR.
• Licensure: Current LPN licensure in the state of practice.
• Certifications: Active CPR Certification.
• Transportation: Valid Driver’s License, active vehicle insurance, and access to a dependable vehicle.
