Advanced Practice In‑Home Provider (90‑Day Assignment) – Orange County, FL

Location: Orange County, FL (Orlando area preferred) 

Schedule: Monday–Friday, 8 AM–5 PM EST 

Assignment Length: 90 days 

Travel: 5–7 visits/day within ~50 minutes of home

Role Summary

This temporary in‑home provider role delivers comprehensive primary and preventive care to patients in their homes or community settings, including assisted living facilities, skilled nursing facilities, and group homes. The position focuses on improving patient outcomes, supporting continuity of care, and providing high‑quality clinical assessments.

Key Responsibilities

  • Perform comprehensive health assessments and physical exams.

  • Diagnose conditions and create individualized treatment plans.

  • Manage chronic diseases such as diabetes, hypertension, and cardiovascular conditions.

  • Prescribe and adjust medications; provide patient education.

  • Deliver preventive care, screenings, and lifestyle counseling.

  • Serve as the patient’s primary care provider of record.

  • Maintain accurate EMR documentation within 48 hours.

  • Collaborate with interdisciplinary care teams.

  • Conduct follow‑up visits and adjust care plans as needed.

  • Participate in after‑hours on‑call rotation as required.

  • Travel reliably to scheduled patient appointments.

Required Qualifications

  • Active, unrestricted state licensure as an NP or PA (no board or clinical restrictions).

  • Certification as FNP, AGNP, or PA‑C.

  • Ability to obtain autonomous provider status within six months (if applicable).

  • Preferred: 2+ years of primary or longitudinal care experience.

  • Active DEA license and current BLS certification.

  • Strong clinical assessment and communication skills.

  • Proficiency with EMR systems.

  • Valid driver’s license (minimum 3 years without restrictions; no DUIs or felony driving convictions in the past 7 years).

  • Reliable transportation and proof of required auto insurance.

  • Ability to pass comprehensive background checks and drug screening.

Preferred Qualifications

  • Experience with HEDIS/STAR measures

  • Home‑based visit experience

  • HCC documentation experience
