Long term Service Care Manager

Pay Range: $30 - $30.10 /hour.  Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities

• Follow up with ALFs regarding member transitions and ongoing needs.
• Communicate with SNF/LTC providers, members, families, caregivers, and other partners.
• Assess member needs and coordinate appropriate services and resources.
• Assist with care and service transition planning.
• Monitor member status, transitions, and outcomes.
• Maintain accurate regulatory documentation and case records.
• Track and document all member transitions and related activities.
• Educate members and families regarding available services, benefits, and care options.
• Collaborate with providers and internal teams to address member needs.

Key Requirements and Technology Experience: 

• 3+ years of experience in Skilled Nursing Facilities (SNF), Assisted Living Facilities (ALF), social work, or a related setting.
• Experience with care transition planning.
• Strong regulatory documentation and case-tracking skills.
• Proficiency with Microsoft Office applications.
• Strong communication skills and confidence speaking with members, families, and healthcare providers.
• Ability to multitask, manage data, and maintain detailed documentation.
• Bachelor’s degree required.
• Degree or professional background in Social Services/Social Work.
