Pay Range: $27 - $30/hour.  Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
 
Key Responsibilities:

• Manage a caseload of 50-70 members with long-term care needs.
• Conduct geriatric assessments and develop care plans in collaboration with members, caregivers, and providers.
• Authorize and coordinate referrals for services, ensuring no gaps in care.
• Monitor service delivery through in-person visits and telephonic contact.
• Assist members with complaints and appeals, ensuring their voices are heard.
• Maintain accurate documentation within 24 hours of service completion.
• Work independently and connect with IT resources as needed.

Key Requirements and Technology Experience:

• 2+ years of Care Management experience, specifically in a field setting.
• Experience managing high caseloads of members aged 65 and above.
• Strong knowledge of Long Term Care Medicaid and Medicare processes.
• Proficiency with electronic medical health records.
• Valid driver's license.
• Experience with discharge planning.
• Familiarity with TruCare software.
