Pay Range: $35.40 - $40/hour .  Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities: - 

• Evaluate the needs of members and recommend effective care plans.
• Develop ongoing long-term care/service plans and identify necessary resources.
• Coordinate communication between members, families, and care providers.
• Provide resource support for employment, housing, and independent living.
• Monitor and document care plans, member status, and outcomes.
• Conduct home and site visits to assess member needs.
• Educate members and families on healthcare options and benefits.
• Offer feedback to leadership on quality improvement opportunities.

Key Requirements and Technology Experience: 

• Key skills: - Additional certifications in case management or related fields.
• 2 – 4 years of related experience in case management or long-term care.
• Bachelor’s degree in a relevant field.
• Delaware/Compact RN license preferred.
• Must have a valid driver’s license and a home workspace.
• Experience in Level of Care Redetermination and Transition of Care.
