Summary: Longmont United Hospital is looking for a locum OB/GYN to help with Urgent May-July coverage for emergent GYN and OB deliveries.
BID DUE DATE: May 18th at 5p EST!
RATES:
Malpractice Must be included in rates.
Orientation ***-225 - half the hourly/callback rate*** kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
On Call (Daily) *** - *** - Per 24 hours to include 4 hours gratis"
24 Hour Holiday Call Rate - ***-*** - Per 24 hours to include 4 hours gratis"
Call Back ***-450 Per hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms.
Holiday Call Back ***-675 Per holiday hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms.
Average worked Callback Hours:Weekday: 4.5 hours; Weekend: 5 hours
Schedule/Availability Requirements:
Site has some flexibility to move or adjust some dates but not all for the 24 hour call shifts

Details:
- 24 hour call (7a-7a)
- Total of 9-10 shifts needed per month for all locums.
- Site will not schedule providers more than 2-3 day stints.
- Setting: Inpatient, Emergency Room, Call Trauma Level III
- FTE: 2.0
- Required Procedures/Job Duties: GYN Emergencies, Obstetrics including C-section.
- EMR: EPIC
- Support Staff: RN
- Reason for Coverage: Gaps in coverage
- Dealbreakers (Do Not Present): Must be willing and able to work 24 hour shifts.
EMR Required Training:
- Credentialing Preapp and full app must be completed and submitted to move provider into "applicant in training" to generate AD account and begin EMR account set up. Once AD account creation begins it can take up to 5 days to create account.
- Module (learn lessons) Must be completed prior to virtual EMR training to attend the training.
- EMR Virtual (PPL) training Only able to attend if Modules are completed before training. Virtual training must be completed at least one week prior to the providers first day.
- First Day Details - Site requires completion of EMR Modules and training, before first day details can be released.
- All candidates must complete the following EMR questionnaire and detail it in the notes section of the candidates submission at time of presentation.

Travel Preferences:
- Preferred Lodging: Providers must be willing to stay at the Hampton Inn - 850 S. Main st., Longmont, Co - ***. When booking they must reference Centura Health or LUH, to receive the prenegotiated rate between the facility and hotel. Providers are required to book at the preferred hotel when room are available. If rooms are not available, they must follow the CommonSpirit Health travel policy.
- Air Travel: Local/drive-in candidates only - Highly Preferred
- Please detail any anticipated travel exceptions, for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Please also indicate the city where provider will be traveling from in the travel section of the provider submission.
Credentialing Information:
- Timeframe: 60 days temps / 120 days full
- Credentialing with other facilities? Yes
- If so, where: LUH, SAN - Not required for coverage just would be good to be privileged at both.
- Frequency of work at each facility: Most if not all with be at LUH.
- Requirements: Self inquiry NPDB report must be provided and CO license or Active IMLC to release preapplication.
- Requirements: Preapp and full app must be submitted within 72 hours of receipt of initial app to allow enough time for AD account creation for EMR requirements. Once both are submitted, provider is moved to "applicant in training" which initiates AD Account creation for provider EMR account and can take up to 5 days before release of EMR training requirements.
