Rates for submission(strictly follow)
Malpractice Must be included in rates.
Orientation ***-220 - half the hourly/callback rate*** kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
Hourly ***-440 - Per hour, not OT
Holiday ***-660 - Must be 1.5x the hourly rate*** Per holiday hour, no OT
Shift type: 8 hour days - with 1 hour lunch Monday- Thursday
Monday- Thursday 8a-5p, Friday 8a-12pm
Schedule: 4-5 days per week full time starting end of June/early July.
Setting: Inpatient/Outpatient ? 100% OP

Required Procedures: 100% OP general cardiology no "procedures"

Certification Requirements: BC in Cardiology
Board Certifications: will you consider Board Eligible: No
Residency requirements/ Fellowship: Cardiology. Must be US/Candadian, CA accredited program - explanation from credentialing team: Residency training programs must be accredited by the Accreditation Counsel for Graduate Medical Education, American Association of College of Osteopathic Medicine, or the Royal College of Physicians and Surgeons of Canada.
Admissions per Day: No Admits/ 100% OP, patients per day avg 20
EMR: Cerner
Support Staff: MA/Clinic Supervisor, RN
Credentialing Timeframe- 60-90 days, Foundation privileges only.
