GUARANTEED HOURS & PAY
- Guaranteed Hours: 40 Hours Per Week
- Pay Rate: $33.45 - $39.03 per hour
REQUIREMENTS
- Active Licensed Practical Nurse (LPN/LVN) License in good standing
- Current BLS certification preferred
- Previous experience in skilled nursing, rehabilitation, long-term care, or related healthcare settings preferred
- Strong clinical assessment and medication administration skills
- Excellent communication and documentation abilities
- Ability to work effectively within an interdisciplinary healthcare team
- Commitment to delivering compassionate, resident-centered care
KEY RESPONSIBILITIES
- Provide direct nursing care in accordance with physician orders and individualized care plans
- Administer medications, treatments, and nursing interventions safely and accurately
- Monitor resident conditions and promptly communicate changes to the healthcare team
- Perform nursing assessments and maintain accurate patient records
- Assist with the development and implementation of individualized care plans
- Collaborate with physicians, registered nurses, therapists, and support staff
- Maintain accurate and timely clinical documentation
- Ensure compliance with infection-control procedures, safety standards, and regulatory requirements
- Educate residents and families regarding treatments and care plans
- Promote resident comfort, dignity, safety, and overall quality of life
