SHIFT DETAILS

Shift Time: 6:30 AM 7:00 PM
Meal Break: 30 Minutes
12.5-Hour Day Shift

GUARANTEED HOURS & PAY

Guaranteed Hours: 36 Hours Per Week
Pay Rate: $25.20 - $29.40 per hour

REQUIREMENTS

Active Licensed Practical Nurse (LPN/LVN) License
Current state licensure in good standing
Graduate of an accredited practical nursing program
Current BLS certification preferred
Previous long-term care, rehabilitation, skilled nursing, or acute care experience preferred

KEY RESPONSIBILITIES

Administer medications and treatments according to physician orders and nursing standards
Monitor patient conditions and promptly report significant changes
Maintain accurate and timely patient records and clinical documentation
Provide direct patient care while ensuring patient comfort and safety
Assist with care planning and implementation of individualized treatment plans
Collaborate with physicians, registered nurses, therapists, and interdisciplinary healthcare teams
Educate patients and caregivers regarding treatments and care plans as appropriate
Ensure compliance with healthcare policies, procedures, and regulatory requirements
Support quality patient outcomes through effective communication and care coordination

Photo badge must be worn at all times
COVID-19 vaccination or approved religious/medical exemption required
Additional vaccine doses must be obtained when required by law
36 guaranteed hours per week
