The LPN will provide direct patient care, administer medications, monitor resident conditions, and collaborate with the interdisciplinary care team to ensure exceptional patient outcomes.

GUARANTEED HOURS- 40 Hours Per Week
- Pay Rate:$33.00 - $38.50 per hour
KEY RESPONSIBILITIES- Administer medications and treatments according to physician orders.
- Monitor residents' health conditions and report changes to the care team.
- Maintain accurate and timely nursing documentation.
- Assist with care planning and implementation.
- Support activities of daily living and promote resident comfort and safety.
- Collaborate with nurses, physicians, therapists, and other healthcare professionals.
- Adhere to all clinical, regulatory, and facility policies and procedures.
ADDITIONAL INFORMATION- Local contract opportunity.
- Skilled Nursing and Long-Term Care setting.
- Evening shift assignment.
- 40 guaranteed hours per week.
- 30-minute meal break included.
- Agency staff must provide a negative COVID-19 test result collected within one week of the contract start date or proof of full vaccination.
- Contract start dates may be delayed pending receipt of required test results.
- Once the assignment begins, staff must comply with the facility's ongoing testing protocols.
CERTIFICATIONS & LICENSURE- Active Licensed Practical Nurse (LPN/LVN) License
