Position Summary

The Social Worker/Care Manager will provide comprehensive psychosocial assessments, care coordination, and discharge planning for hospitalized patients. The role focuses on identifying high-risk patients, coordinating post-acute services, preventing readmissions, supporting appropriate length of stay, and facilitating transitions between levels of care.

Key Responsibilities

  • Complete comprehensive initial psychosocial assessments upon admission.
  • Screen patients for high-risk indicators and discharge planning needs.
  • Perform ongoing reassessments throughout the hospital stay.
  • Coordinate care with multidisciplinary hospital teams.
  • Coordinate intra-hospital and inter-hospital transitions of care.
  • Develop and facilitate discharge plans.
  • Provide patient and caregiver education.
  • Arrange appropriate aftercare and post-acute services.
  • Collaborate with commercial payers when needed.
  • Address high-utilizer patients and identify root causes.
  • Communicate relevant patient information to physicians and healthcare team members.
  • Support length-of-stay reduction and readmission prevention.
  • Assist with denial reduction and patient satisfaction initiatives.
  • Complete required documentation in the EMR.
  • Participate in Performance Improvement activities.

Requirements

  • Master of Social Work (MSW) – Required
  • Active New York State LMSW or LCSW – Required upon hire
  • NYS PRI/Screen Certification – Required within 90 days
  • 2 years of acute-care hospital or related experience preferred
  • Experience with Electronic Medical Records (EMR)
  • Care management/discharge planning experience preferred.
