Job Summary

We are seeking a skilled and compassionate Physical Therapist to provide evaluation and rehabilitation services to patients in a rehab facility. The PT will develop individualized treatment plans focused on improving mobility, strength, balance, and overall functional independence.

Responsibilities

  • Evaluate patients and assess mobility, strength, balance, and functional limitations.
  • Develop individualized treatment plans based on patient needs and goals.
  • Provide therapeutic exercises, gait training, mobility training, and functional activities.
  • Assist patients with improving strength, coordination, balance, and range of motion.
  • Monitor patient progress and modify treatment plans as needed.
  • Document evaluations, treatment sessions, progress notes, and discharge recommendations accurately.
  • Educate patients and families on exercises, mobility, and safety techniques.
  • Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals.
  • Follow facility policies, infection-control procedures, and patient-safety standards.
  • Participate in care planning and discharge planning as required.

Qualifications

  • Current California Physical Therapist (PT) license in good standing.
  • Graduate of an accredited Physical Therapy program.
  • Current BLS/CPR certification preferred.
  • Previous experience in rehabilitation, SNF, LTC, or post-acute care preferred.
  • Strong clinical, communication, documentation, and organizational skills.
  • Ability to work effectively with patients with varying levels of mobility and functional ability.
