HonorVet Technologies is seeking a Physician Assistant Family Practice for a local contract job in Middletown, New York.

Position Overview:
- Employment Type: Full-Time, Permanent
- Locations: Port Jervis / Middletown, Dutchess County, New York
- Specialty: Family Practice / Geriatric Care
- Schedule: No weekends or calls
- Patient Population: Geriatric / Older Adults
- Base Salary: Up to 170,000 per year
Port Jervis / Middletown, NY – Full-Time NP
- Monday through Friday schedule.
- Provider would work between our Port Jervis and Middletown locations
- Full-time schedule with consistent assigned days/locations
Dutchess County, NY – Full-Time NP
- Monday through Friday schedule.
- Coverage within the Dutchess County area, which may include Poughkeepsie, Wappingers Falls, Beacon, and surrounding locations
- Provider must be comfortable traveling between assigned facilities (although typically never in the same day)
Minimum Requirements:
- Active and unrestricted New York State professional license as an NP
- Active DEA registration
- Active Medicare and Medicaid enrollment
- Current CAQH profile
- For Nurse Practitioners, completion of the required 3,600 hours of collaborative practice experience for independent practice in New York
- Ability to successfully complete our credentialing process
