Registered Nurse (RN) – Community Care

Dayton VA Medical Center, Dayton, OH  |  Full-Time |  Monday–Friday, 7:30 AM–4:00 PM (no weekends, no federal holidays)

 

Position Overview

We are seeking a Registered Nurse to support the Community Care program within Patient Business Service at the Dayton VA Medical Center. This is an administrative, non-bedside RN role focused on reviewing and processing community care consults and referrals for Veterans, coordinating with VA and community providers, and ensuring timely, accurate care coordination. Candidates should have a background in med-surg or primary care nursing and be comfortable working in an office-based, computer-intensive environment using Oracle Health Share Referral Manager (HSRM) and VA's electronic health record.

 

Key Responsibilities

  • Review, triage, and process Community Care consults and referrals for assigned Veteran patients.
  • Coordinate care and communicate directly with VA and community-based providers to determine appropriate course of treatment.
  • Document all care coordination activity accurately and timely in CPRS/the VA electronic health record.
  • Utilize Oracle Health Share Referral Manager (HSRM) to track and manage referral status and disposition.
  • Participate in daily team huddles and weekly team meetings with the assigned Patient Aligned Care Team (PACT).
  • Follow all established patient safety and infection control standards; report patient safety events within 24 hours.
  • Maintain compliance with all Privacy Act, HIPAA, and VA confidentiality requirements.
  • Communicate proactively with the COR regarding any obstacles to meeting consult/referral turnaround requirements.

Qualifications & Experience

  • Current, active, unrestricted Registered Nurse license issued by the Ohio Board of Nursing.
  • Graduate of an ACEN- or CCNE-accredited school of professional nursing.
  • Current BLS certification and current CPR certification (American Heart Association).
  • Minimum of one (1) year of direct patient care experience within the last three (3) years; med-surg, primary care, or care coordination background preferred. VA experience desirable.
  • Strong computer proficiency; experience with Oracle HSRM and/or VA CPRS a plus.
  • Excellent verbal and written communication skills for collaborating with providers inside and outside the VA.
