ob Summary:
The LTC RN provides nursing care to residents in a long-term care or skilled nursing facility. The RN is responsible for assessing residents, administering medications and treatments, monitoring changes in condition, coordinating care, and supervising nursing staff.
Key Responsibilities:
- Assess residents and monitor their health and changes in condition.
- Administer medications, treatments, and other nursing interventions.
- Develop, update, and follow individualized care plans.
- Monitor vital signs, symptoms, wounds, pain, and overall resident condition.
- Communicate changes in condition to physicians/providers and families.
- Document assessments, medications, treatments, and resident care accurately.
- Coordinate care with LPNs/LVNs, CNAs, physicians, therapists, and other healthcare staff.
- Supervise and delegate appropriate duties to LPNs/LVNs and CNAs.
- Assist with admissions, transfers, and discharges.
- Provide patient and family education and emotional support.
- Respond to emergencies and provide appropriate nursing care.
- Maintain resident safety, dignity, comfort, and quality of life.
Requirements:
- Active RN license in the applicable state.
- BLS certification preferred/required based on facility.
- LTC, skilled nursing, geriatrics, or similar nursing experience preferred.
- Strong assessment, documentation, communication, and clinical skills.
