Job Details:
• Schedule: Monday-Friday, 8-hour shifts between the hours of 7:00 AM and 6:00 PM
• Guaranteed Hours: 40 hours per week
• On-Call Requirement: At least 1 night per week and 2 nights during one week of the schedule rotation, plus weekend on-call coverage
• Location: San Marcos, TX
• Job Type: Contract
• Pay Range: $52-$54/hour
*Pay offered is based on experience, expertise, credentialing, and education.

 

Duties:
• Provide comprehensive hospice nursing care to patients and families.
• Perform patient admissions, routine visits, and end-of-life care visits.
• Manage and coordinate patient care plans in collaboration with the interdisciplinary team.
• Complete deaths visits and provide appropriate documentation and support.
• Complete accurate and timely documentation in accordance with agency standards.
• Participate in required on-call rotations, including nights and weekends.

 

Requirements:
• Active RN License
• Hospice experience required
• Recent HCHB (Homecare Homebase) experience required
• Ability to perform admissions, deaths, GIP care, and routine patient visits independently
