PRIDE Health is seeking a local contract nurse RN Home Health for a local contract nursing job in Mountlake Terrace, Washington.
Pride Health is hiring a RN - Home Health to support our client's healthcare facility in King County and Snohomish County, WA.
Job Details:
Schedule: Monday-Friday | 8:00 AM-4:30 PM | On-Call/Weekend: 5 Weekdays and 2 Weekends Minimum
Location: King County and Snohomish County, WA; Mileage reimbursed between patient visits.
Job Type: Contract
Pay Range: $56-$60
*Pay offered is based on experience, expertise, credentialing, and education.
Duties:
• Initiate, develop, implement, and revise individualized plans of care in collaboration with physicians and interdisciplinary healthcare teams.
• Perform initial and ongoing nursing assessments, including accurate and timely OASIS assessments and medication reconciliation, to determine patient eligibility and medical necessity.
• Provide skilled nursing care according to the plan of care while supervising and directing home health aides and LPNs in compliance with State and federal regulations.
• Maintain accurate and timely documentation while coordinating care and communicating with physicians, patients, families, clinicians, and agency staff to ensure quality care and positive patient outcomes.
Requirements:
• License(s): RN license in Washington State or Multi-State
• Certification(s): BLS
• Years of experience: 1+
• Skills/Specialties/Technology: Home Health experience preferred; HCHB/PointCare experience preferred; OASIS documentation experience preferred; strong clinical assessment, care planning, communication, organization, and time management skills.
