Pride Health is actively hiring a Registered Nurse (RN) to join our client's Home Health Team.

 

Location: Mountlake Terrace, WA 98043

Pay Rate: Up to $61.00/hour on W-2

Schedule: Monday–Friday, 8:00 AM–4:30 PM + On-Call

Coverage Area: King County and Snohomish County

Weekly Productivity: Minimum 30 points

On-Call/Weekend Requirement: Yes – 5 weekdays and a minimum of 2 weekends

Patient Care Setting: Home Health

Mileage: Reimbursed between patient visits, from the first patient to the last patient

Position Overview

  • Pride Health is hiring an experienced Registered Nurse (RN) – Home Health to join our client's healthcare team in Mountlake Terrace, WA. The RN will provide comprehensive skilled nursing care to patients in their homes across King and Snohomish Counties, ensuring quality outcomes, accurate documentation, and coordinated patient care.
  • The ideal candidate will have home health experience, strong clinical assessment skills, and proficiency with home health EMR systems such as HCHB or PointCare.

Key Responsibilities

  • Clinical Assessment & Care Planning: Conduct initial and comprehensive nursing evaluations, complete OASIS assessments, determine home health eligibility and medical necessity, and develop and revise individualized care plans in collaboration with physicians and healthcare professionals.
  • Patient Care Delivery: Provide high-quality skilled nursing services within scope of practice and infection control standards, administer care according to the plan of care, and supervise HHAs and LPNs/LVNs as required by state and federal regulations.
  • Documentation & Compliance: Complete accurate and timely OASIS assessments, medication reconciliation, patient visit documentation, and EMR synchronization in accordance with agency policies, payer requirements, and LHC guidelines.
  • Communication & Care Coordination: Coordinate with physicians, patients, families, schedulers, and interdisciplinary teams regarding patient needs, visit assignments, orders, schedule changes, and continuity of care.
  • Quality & Professionalism: Participate in case conferences, performance improvement initiatives, survey readiness, utilization management, and patient safety programs while maintaining punctuality, compliance, and professional standards.
  • Teamwork, Productivity & On-Call: Meet the minimum productivity expectation of 30 points per week, participate in weekday and weekend on-call rotations (minimum 2 weekends), assist with staff orientation, and maintain flexibility to support patient care needs.

Required Qualifications

  • Unencumbered Registered Nurse (RN) license in Washington State or a valid Multi-State/Compact RN license, as applicable.
  • Current Basic Life Support (BLS) certification.
  • Strong clinical assessment, documentation, and patient care coordination skills.
  • Ability to work independently in a home health environment.
  • Willingness to participate in on-call and weekend rotations.

Preferred Qualifications

  • Previous Home Health nursing experience.
  • Experience with Homecare Homebase (HCHB) and/or PointCare EMR systems.
  • Knowledge of OASIS assessments and home health documentation requirements.
  • Strong communication, teamwork, and organizational skills.
