Job Details: 
Schedule: Monday–Friday | 8:30 AM–4:30 PM
Location: Forty Fort, PA
Coverage Area: Luzerne County, PA
Contract Length: 15+ Weeks (Possibility to Extend)

Local Pay Range: $52.00 - $55.00/hour (W-2) Plus Mileage reimbursement
Mileage Reimbursement Rate: $0.50/ mile
*Pay offered is based on experience, expertise, credentialing, and education.
 
Duties: 

  • RN Case Manager assigned to a specific territory, responsible for managing and coordinating patient care within the designated area.
  • Performs skilled routine visits, recertifications, discharges, Start of Care (SOC), and Resumption of Care (ROC) visits.
  • Maintains 100% patient-care focus; each case manager manages their own territory and provides coverage for other case managers during their days off.

 

Requirements: 

  • License(s): Current and unrestricted RN licensure in state of practice
  • Certification(s): Current CPR Certification or ability to complete within 90 days of hire
  • Years of experience: 1+ years of Home Health experience and Homecare Homebase (HCHB)
