Registered Nurse (RN) – Home Health Case Manager


 

Pay Rate: Up to $58.00/hour (W-2)
Weekly Hour Guarantee: 36 hours/week
Productivity Expectations: 30 points/week
Schedule: Monday to Friday, 8:00 AM–4:30 PM
On-Call/Weekend Requirement: No
Mileage Reimbursement: Yes, per contract/company policy

Patient Care Setting: Home Health

Position Overview:

  • We are seeking an experienced Registered Nurse (RN) Case Manager to provide skilled nursing care to patients in a Home Health setting. The RN Case Manager will be responsible for coordinating and managing patient care, completing assessments, developing individualized Plans of Care, collaborating with the interdisciplinary team, and ensuring high-quality patient outcomes.

Key Responsibilities:

  • Serve as the primary RN Case Manager for an assigned caseload of Home Health patients.
  • Conduct comprehensive nursing assessments and evaluate patients' clinical and functional needs.
  • Develop, implement, and update individualized Plans of Care (POC) based on patient needs and physician orders.
  • Provide skilled nursing services in accordance with the established Plan of Care and applicable standards of practice.
  • Monitor patient progress and identify changes in condition requiring intervention or escalation.
  • Coordinate care with physicians, therapists, social workers, aides, and other members of the interdisciplinary healthcare team.
  • Educate patients and caregivers regarding medications, disease management, treatment plans, safety, and self-care.
  • Perform medication reconciliation and ensure accurate medication management.
  • Complete required clinical documentation accurately and within established timelines.
  • Maintain compliance with Home Health, regulatory, payer, and organizational requirements.
  • Communicate effectively with patients, families, physicians, and other healthcare professionals.
  • Promote continuity of care and help ensure patients receive safe, effective, and patient-centered services.
  • Travel to patients' homes throughout the assigned coverage area.

Required Qualifications:

  • Active Registered Nurse (RN) license required.
  • Previous nursing experience required.
  • Strong clinical assessment, communication, organization, and documentation skills.
  • Ability to manage a patient caseload independently.
  • Ability to provide skilled nursing care in patients' homes.

Preferred Qualifications: Previous Home Health experience strongly preferred. HCHB (Homecare Homebase) experience preferred. Experience with Home Health care management, OASIS, and Plan of Care development is a plus. Strong ability to work independently and coordinate care across multiple disciplines.
