Pride Health is hiring a Travel Registered Nurse (RN) – Home Health to support our client's healthcare facility in Warren County, Kentucky
 
Job Details: 
 

Job Title: RN Home Health 
Schedule:  Monday–Friday, 8:00 AM–4:30 PM; on-call 2–3 nights/week and 1 weekend every 3–4 weeks
Location: Warren, Edmonson, Hart & Butler Counties, KY  
Float / Coverage Area: Four-county service area
Job Type: Local contract 
Contract Length: 13 Weeks 

Mileage Reimbursement Rate (if applicable): $0.48/ mile
*Pay offered is based on experience, expertise, credentialing, and education.  

Duties: 

  • Perform comprehensive OASIS assessments, admissions, recertifications, and other required home health visits.
  • Provide routine skilled nursing care and monitor patients' conditions throughout the course of treatment.
  • Develop and implement individualized plans of care and communicate changes in patient status to physicians and the interdisciplinary care team.
  • Educate patients and caregivers on medications, disease management, safety, and home care needs.
  • Complete timely clinical documentation while effectively managing a daily field schedule across the assigned service area.

Requirements: 

  • Education: Nursing degree from an accredited nursing program
  • License(s):  Active RN License; must be authorized to practice in Kentucky
  • Certification(s): Active RN License
  • Years of experience: Previous home health experience required
  • Skills/Specialties/Technology: OASIS assessments, skilled nursing, patient assessment, care planning, documentation, time management, care coordination, and ability to work independently in a home health setting
