Job Summary
The Primary Care Nurse is a clinically competent practitioner of pediatric, family-centered nursing care. This individual is responsible for providing safe, high-quality patient care while collaborating with patients, families, providers, and interdisciplinary team members to ensure positive outcomes.
Key responsibilities include:
- Utilizing the nursing process through assessment, planning, implementation, and evaluation.
- Managing care for patients with moderate to complex healthcare needs.
- Collaborating with care teams to develop and update patient care plans.
- Providing patient and family education and coordinating care needs.
- Maintaining accurate and timely clinical documentation.
- Complying with all professional, regulatory, and organizational standards.
- Participating in patient care rounds and practice improvement initiatives.
- Demonstrating professionalism and a commitment to delivering an exceptional patient experience.
- Rotating to other Care Network sites within the designated cluster, as needed.
Required Skills & Experience
- Basic computer proficiency, including use of common hardware and peripherals.
- Working knowledge of Microsoft Office applications (Word, Excel, PowerPoint, Teams/Skype).
- Ability to access and review patient information within clinical information systems.
- Experience utilizing clinical data and quality improvement tools.
- Knowledge of confidentiality, privacy, and information security requirements.
- Strong clinical judgment, communication, and organizational skills.
- Experience caring for patients with moderate to complex health conditions.
