Pay: $50/hour

Shift: Monday - Friday | 8:00 AM - 4:30 PM

  

Minimum Requirements

  • Active RN License
  • Active BLS Certification
  • Minimum of 2 years of nursing experience

 

Job Summary

The Primary Care Nurse is a clinically competent practitioner of pediatric, family-centered nursing care. This individual is responsible for providing safe, high-quality patient care while collaborating with patients, families, providers, and interdisciplinary team members to ensure positive outcomes.

 

Key responsibilities include:

  • Utilizing the nursing process through assessment, planning, implementation, and evaluation.
  • Managing care for patients with moderate to complex healthcare needs.
  • Collaborating with care teams to develop and update patient care plans.
  • Providing patient and family education and coordinating care needs.
  • Maintaining accurate and timely clinical documentation.
  • Complying with all professional, regulatory, and organizational standards.
  • Participating in patient care rounds and practice improvement initiatives.
  • Demonstrating professionalism and a commitment to delivering an exceptional patient experience.
  • Rotating to other Care Network sites within the designated cluster, as needed.

 

Required Skills & Experience

  • Basic computer proficiency, including use of common hardware and peripherals.
  • Working knowledge of Microsoft Office applications (Word, Excel, PowerPoint, Teams/Skype).
  • Ability to access and review patient information within clinical information systems.
  • Experience utilizing clinical data and quality improvement tools.
  • Knowledge of confidentiality, privacy, and information security requirements.
  • Strong clinical judgment, communication, and organizational skills.
  • Experience caring for patients with moderate to complex health conditions.
